Support at home and in the community
Services are individualized according to assessed needs, authorization and the person’s support plan.
Group home residential
Twenty-four-hour support in small community homes, including assistance with routines, personal goals and household life.
Apartment-style support
Individualized support in smaller apartment settings that encourage independence, privacy and self-direction.
Sponsored residential
Family-style support in a trained sponsor’s home. Three sponsor hosts are currently accepting placement referrals.
Community engagement
Support to participate in recreation, volunteering, relationships and other meaningful activities in the community.
Community coaching
Individualized skill building in community settings based on the person’s interests, goals and support needs.
Nursing and health coordination
Coordination of medical appointments, health instructions, chronic-condition supports and authorized nursing oversight.
